Hormone therapy when it’s the right fit for you. The form will be chosen to fit your preferences, whether that means a patch, a gel, a spray, or a pill.
Weight gain gets treated as biology rather than just willpower. That can include GLP-1 medication when it’s right for you. Dr. English will also build you a customized nutrition plan that fits your goals and preferences.
Sleep, mood, and focus get treated like the medical problems they are. Medication is part of that when it fits, and they get the same attention as your hormones.
Low libido, dryness, and pain during sex get treated as the medical issues they are. Dr. English raises it at your first visit, so you never have to.
Your career, your family, your schedule. These are what decide which treatments you can realistically stay on, and which ones would just add pressure to a life that’s already full. You get a plan that’s practical and fits into your life.
You start with a full hour with Dr. English, who will have read your records and your history before you meet. Every area perimenopause touches gets covered in one conversation, nothing ever feels rushed.
You leave that initial consultation with a written plan in plain language. It explains what’s being treated, in what order, how, and why.
Because your body keeps changing throughout perimenopause, and there is no one-size-fits-all solution, regular check-ins are important. Your pre-scheduled adjustment visits with Dr. English allow her to understand how you’re feeling and adjust your plan based on how your body is actually responding.
You get unlimited secure messaging straight to Dr. English between adjustment visits, so a question on a Tuesday gets answered on a Tuesday.
When your history calls for testing, Dr. English orders the labs, bills them to your insurance, and explains what they say in plain language. One less thing for you to track down or decipher.
Her office takes on as much of the paperwork as it can. Insurance questions, lab forms, pharmacy hassles. Whatever is left, they walk you through step by step. You’re not left decoding systems alone.
When something calls for a specialist, you get appropriate recommendations and facilitated communication in pelvic floor therapy, nutrition, psychotherapy, and sleep. Dr. English stays your primary perimenopause doctor throughout.

"Yesterday was my first visit with Dr. English and I can’t even express how happy I am. She spent an hour with me to really get to know me. I have heard only great reviews, and now I see why. What a delightful experience so far!"
"I’m so lucky to have found Dr. English. She is so caring, competent and invested in her patients health and well being."
You’ve had the bloodwork. “Everything’s normal,” they said.
You’ve mentioned the exhaustion, the brain fog, the nights you’re wide awake at 3am for no reason. “That’s just stress.” “That’s just being a woman in your 40s.”
You’ve watched the weight climb even though nothing about your diet or your workouts has changed. You’ve felt your patience disappear, your libido disappear, your sharpness disappear, and wondered privately, is this just what’s left of me now?

Hi, I'm Dr. Jacquelyn English, DO
Board-certified in Family Medicine. NAMS-certified in menopause. Board-certified in Obesity Medicine.
The credential that probably matters most to you is this:
I know what it's like to lead a very busy life at full speed, and to need my body and brain to keep up.
I built and run my own private medical practice. I'm a professor teaching the next generation of medical students. I serve on the board of a non-profit that I care deeply about. And I'm raising two beautiful kids as a single mom.
So when a woman tells me she can't afford to lose a step right now, not at this point in her life, not when she's worked this hard to get here, I don't need that explained to me.
And unfortunately, that's exactly when perimenopause tends to show up.
I spent years watching it happen to my patients.
The most capable women I knew, sharp, driven, doing everything right, would come in with the same cluster of symptoms all at once: the 3am wakeups, weight gain they can't get on top of, bursts of rage out of nowhere, brain fog right when they needed to be at their best, a libido that quietly disappeared.
And all too often, they'd been told by their doctors that their labs were "normal." That it was "just stress". "Just normal aging." It's not.
And being dismissed at the moment you most need to be firing on all cylinders isn't just frustrating, it's a real threat to the life you've built.
So, I built Clarity Physician Partners to do it all differently.
I work with you directly on a plan that treats all five systems perimenopause touches: hormones, metabolism, brain and mood, sexual health, and the load you're already carrying, together, the way they're actually happening to you. Not one symptom at a time.
Not a rushed fifteen minute office visit. Not set it and forget it. Not "let's wait and see."
Perimenopause is not a disease or a diagnosis. It's a phase of life.
You don't need to be cured. You just need to be heard. Then optimized.
I'd love to partner with you and work together to get you back to feeling like yourself again.

The 12-week Perimenopause Reset program fee is paid out-of-pocket. When labs are ordered, they're billed to your insurance. Medication and pharmacy costs are separate and vary by your insurance and pharmacy.
The 12-Week program is $2,388. Payment plan options are available. Kristina, our Care Coordinator, will walk you through all of the options on your discovery call, before you commit to anything.
No. First, Dr. English reviews your symptoms, history, and risk profile. She prescribes only when it's medically right for you.
Perimenopause often starts while your periods still look regular. Symptoms can show up years before your last period. If your body has changed and nobody can tell you why, that's reason enough for a real evaluation.
Yes, and you should. Dr. English handles your perimenopause care. Your OBGYN still does your annual exam and screenings. If something calls for a specialist, you get recommendations, and Dr. English stays your perimenopause doctor throughout.
Yes. Dr. Jacquelyn English, DO, MPH, is board certified in menopause and obesity medicine, and fully licensed in New York State. On video, she can evaluate you, diagnose you, and prescribe for you, just like an office visit. She will have read your chart before you arrive. Prescriptions go to your own pharmacy.
This is why we have the "Right-Fit" Guarantee. The first visit is the trial. If you and Dr. English decide together that this isn't the right care for you, everything you've paid gets refunded. And your written plan is yours to keep either way.
Yes. In perimenopause, hormone levels swing week to week, so one blood draw can look fine on your worst month. Dr. English doesn't need a failed lab to take your symptoms seriously. She works from your symptoms, your history, and your risk profile. Labs are ordered when your history calls for them, then explained in plain English.
The visit starts from a different question: what is actually driving this? Dr. English reads your records and symptom history before you arrive. Then you get a full hour, and everything perimenopause touches gets assessed in one conversation. You leave with a written plan, so you always know what you're doing and why.
Yes. The 12-week program includes unlimited secure messaging, straight to Dr. English. Your questions get answered when they come up, not saved for the next visit.
The other online services connect you to a a clinician, often through a questionnaire, who is often not a perimenopause specialist, or even an actual doctor. Here, you work directly with Dr. English at every visit. She builds your plan on your history, then adjusts it herself as your body responds. She only takes on ten new patients a month so every patient gets an intimate level of care.
After the 12-weeks ends there are multiple options for continuing to work with Dr. English.
